The completion of this application requires partnering companies to submit copies of the following Insurance and Licensing Certificates that meet specific requirements.

• All insurance coverage must be a "B" classification or above.
• Workers' Compensation Insurance or Employer's Liability Insurance (minimum of $100,000 per occurrence) Certificate.
• Comprehensive General and Vehicular Liability insurance for claims and damages due to bodily injury, property damage, etc. Minimum limits are $1,000,000 for each person, $1,000,000 for each incident involving bodily injury and $1,000,000 for each incident involving property damage.
• Professional Liability Insurance including Errors and Omissions coverage with limits of $2,000,000 per occurrence.
• Current business license.
• Completed W9 certificate. (Request for Taxpayer ID form)
• Certificate of Incorporation
